Carrier Liability - Limited

Carrier's liability for loss or damage to any article(s) or part thereof for which the charges are determined by FAK (Freight of All Kinds) class or Exception class is limited to the (1) actual cost of the goods lost, damaged or destroyed; (2) exclusions or limited liability provisions of the Bill of lading; (3) $1.00 per pound per package.

Galactic Transport Miscellaneous Fees

Additional Pick-up: $30.00 per
Mileage Rate: $2.75 per mile
Labor (drayage): $36.00 up to one hour
Waiting fee: $24.00 per hour (15 minutes allowed for load / unload)

Fuel Surcharge

A fuel surcharge adjusted by the Rocky Mountain Region weekly diesel price

Note A - Subject to minimum fuel adjustment charge of $2.00 per shipment when the DOE Diesel Fuel Price Index is at least 115 cents per gallon.

Note B - All shipments will be subject to a fuel adjustment (surcharge) based on a weekly fuel lprice index. The index shall be the Department of Energy's National Average Diesel Fuel Price Index, after 4:00PM on Monday of each week. If Monday falls on a holiday, the index for the next business day that is not a holiday will be used to determine the applicable fuel surcharges. The surcharge for each (Monday-Sunday) will be based on Mondays DOE index of the previous week.

Note C - Average Price on 05/18/2009 was $ 2.237. The fuel surcharge for the week is 13.7% and adjusts by .1% for every one cent ($.01) change in diesel price next Monday.
